he needs to go see a family physician/clinic doctor before he can see a specialist, since in most cases referral is required for a specialist (you can not really make appointment with specialist yourself, a doctor\'s office will make it for you).
if one doctor doesn\'t refer him to a specialist, try another one. emphasize how severe the symptoms are and how his life is affected greatly by them.
