Apply EI

Will get my mat leave in January next year. How can I apply EI and where? Anybody has some idea? Thanks

There are two forms on the website.
\"Notice of Appeal to Board of Referees\" form
\"Notice of Appeal to the Umpire\" form

For EI applying, we just need the first form which is \"Notice of Appeal to Board of Referees\", or both forms?

But these two are appeal formS, only when your EI beniefits have been refused. What should be the correct form to fill in to apply EI?

可以网上申请,也可以去附近的Service Canada Centre当面申请.要是有些地方搞不懂还是当面去申请比较好,有问题可以直接问人.

听说是要在停止工作40天内申请,不然对benefit有影响(如果有错,欢迎指正)
要注意的是申请时要带好一年内所有雇主出的ROE(record of employment)
ROE要经过雇主的会计出,看不同公司需要几天到几周不等.这个比较麻烦
如找不到雇主或者其他原因不能拿到ROE,可以跟Service Canada讲,他们会帮你办.

很感谢楼上的。我问过公司的HR，他们说等我走的那天就会给份ROE，到时候就可以去申请。也可以现在申请，到时候自己不能去的话，直接叫老公把我的ROE交去就好了。
